The SYNRGY program includes a fully insured Hospital Indemnity policy issued and administered by an independent insurance carrier, together with the SYNRGY application platform which provides engagement tools and access to services. This policy is a fixed indemnity insurance policy and is not major medical health insurance. The policy pays limited cash benefits for qualifying events according to the terms of the applicable insurance policy. Payments are not based on the amount of your medical bill, and policy limits may apply. Eligibility for coverage, benefit payments, and claim determinations are made solely by the insurance carrier in accordance with the insurance policy. SYNRGY provides the platform used to support engagement with available services and may transmit certain activity information to the insurance carrier or its designated administrator for claims review. SYNRGY does not determine eligibility for insurance benefits or make claim decisions. This policy is not a substitute for comprehensive health insurance and may not satisfy requirements for minimum essential coverage under the Affordable Care Act.
